Use of CCTS Facility & Resources
CCTS resources are available to investigators from any department at the University of Utah. The Principal Investigator must have a faculty appointment at the University of Utah. If the protocol requires patient admission, the Principal Investigator or at least one co-investigator must have admitting privileges to the University of Utah Hospital and Clinics, or if the study is to take place at Primary Children's Hospital, admitting privileges at PCMC are necessary. Interdisciplinary and collaborative research endeavors are encouraged.
Institutional Review Board (IRB)
IRB approval is required for all protocols. The application for IRB approval should be made before submission of your CCTS protocol. Patients cannot be enrolled in CCTS sponsored studies unless IRB approval has been granted.

Before beginning your CCTS protocol application, please read the following instructions carefully.
Protocol Application Deadlines:
All protocols should be e-mailed to Lynette Holman by the date designated on the application deadlines page.
Submitting Protocol Application:
E-mail Lynette Holman with the following file attachments:

» IRB Application File
» IRB Protocol Summary File
» All Consent Form Files
If IRB approval has been received, submit one hard copy of the IRB approval notice.

Consent Forms
The NIH requires that the following information be obtained from each research subject. However, the subject can decline to provide any demographic information.
» Gender
» Date of Birth
» Ethnicity
» Respectfully decline to provide demographic information.
Please include a request for this information in the signature section of the consent form.
